| Stephanie Jordan joined SUHI in 2016 as a Community Health Worker (CHW) I for Health and Healing for Life: Preventing Cancer and Promoting Early Detection through Screens and Education. Stephanie and team provide health education to women and men in communities throughout Sinai Chicago’s service area. Stephanie was promoted to a CHW III in 2020. Stephanie also works on the CHARM pilot (CHW Assistance Resource in Mammography), screening patients for social determinants of health, assisting mammography staff with preparing patients for exams, and escorting patients throughout the process. She maintains ongoing communication with patients.
Stephanie has over 20 years of experience in healthcare working in customer service. She is a lifelong Chicagoan, born and raised in the North Lawndale community. |
