Rachel Morris joined SUHI in 2021 and currently serves as the Director for CROWD (Center for CHW Research, Outcomes, and Workforce Development). Rachel leads business development, strategy, and consulting efforts to ensure CROWD’s training offerings and delivery methods meet the needs of CHWs’ expanding roles within health systems and community-based organizations. She manages the CROWD team, including reviewing and improving processes and prioritizing professional development opportunities. Rachel co-developed and implemented the care coordination model for one of the largest Illinois Healthcare Transformation Collaborative grants awarded in the state, an undertaking that increased access to health care throughout the South Side.
Rachel has significant experience developing and implementing population health strategies with hospitals and community health providers. Prior to joining SUHI, she served as the Chicago Executive Director at VillageMD, a value-based primary care company that partners with community-based providers. Rachel oversaw population health and care management teams in the execution of strategic and operational priorities to improve patient outcomes at optimized costs in value-based contracts with Medicare, Medicaid, and commercial payers. |
