| Madeline Woodberry joined SUHI in 2014 as a Community Health Worker (CHW) for a HUD-funded, adult, in-home asthma program. She currently serves as a Senior Clinical CHW on the CHW Assistance Program (CAP) inpatient and outpatient at Mount Sinai and Holy Cross Hospitals, where she specializes in supporting patients with behavioral health challenges. Madeline has worked on many projects including the Asthma CarePartners program and Courage to Quit tobacco treatment program, and brings over 30 years of experience assisting others by working in childcare, customer service, and construction inspection. |
