Sinai Community Health Survey 2.0

The Sinai Community Health Survey is the largest community-driven health survey ever to be conducted in Chicago. The second iteration, conducted in 2015-16, provides hyper-local data on health-related determinants and outcomes for nine diverse Chicago communities. The survey, developed with a community advisory board, includes approximately 500 questions on child and adult health.
