Health and Healing for Life (HHL)

Health and Healing for Life (HHL) is a CHW-led community outreach and navigation model that educates community members about breast, cervical, and colorectal cancers and screenings; provides navigation to obtain free/low-cost screenings and preventive services; and provides follow-up resources for participants with barriers to accessing services. HHL has provided breast health education and navigation to routine cancer screenings for over 17 years, educating over 30,000 individuals on the importance of breast health and navigating to over 7,000 mammograms, establishing HHL as a model for community health care navigation and contributing to the reduction of inequities in breast cancer mortality in Chicago. The fundamental goal of HHL is to eliminate the health inequities that harm communities of color on Chicago’s West and South Sides by preventing cancer and reducing cancer mortality.
