CHW Assistance Resource in Mammography (CHARM)

Launched in 2024, CHARM is a pilot project to embed CHWs in Sinai Chicago’s mammography departments. CHWs work as integrated members of clinical teams, connecting personally with patients, ensuring the clinic space is a welcoming place with people who speak their language, and communicating with patients to preemptively identify any concerns. CHWs work with mammography patients providing health education and needs assessments, identifying and addressing patient concerns, screening for social needs, assisting with registration, locker room station, completing a breast health risk questionnaire, and providing resources. The CHWs also help with reminder calls, referrals, and in addressing any barriers to care.
