Population Health Career Pathways Program
Are you interested in entering the healthcare workforce as a community health worker (CHW)? SUHI’s Population Health Career Pathways Program offers free training for CHW certificates in core skills, behavioral health, health education and public health. Participants in the program receive paid stipends, career coaching and more.

Population Health Career Pathways Program
The Population Health Career Pathways Program (PHCPP) explores an alternative path to developing and strengthening a healthcare workforce focused on population health services while simultaneously providing individuals from communities with limited access to opportunity an avenue for economic advancement. Through these career pathways, the program expands access to healthcare careers for Latinx individuals and others who have historically encountered barriers to entering the field.
Three specialty career pathways are offered:
- Behavioral Health (BH): the BH curriculum focuses on teaching students’ evidence-based mental health interventions and basic levels of care to patients with depression, anxiety, PTSD, and substance use disorders. Students also participate in outreach training and trauma and mental health education. They learn about crisis prevention and benefits of early access to mental health support services.
- Health Education (HE): the HE curriculum focuses on chronic diseases such as asthma, diabetes, and cancer, and on teaching methods and facilitation skills. CHWs’ health education work has been proven to be effective in reducing hospital admissions and increasing patient satisfaction in services.
- System Navigation (SN): the SN curriculum prepares students to be knowledgeable in Medicaid and Medicare. They learn to provide patients the support they need to navigate complex health and billing systems, and the skills to guide and connect patients to resources available in their community.

- Mentoring and career coaching
- Career exploration services
- Job readiness and digital literacy workshops
- Job placement assistance and participation in specialty job fairs
- Access to paid apprenticeships
Program Eligibility:
- 18 years or older
- Have a high school diploma or GED
- Possess a computer/laptop and have reliable internet connection
- Committed to a career in the healthcare industry
- Have a Gmail account
- Be available 7 hours a week for hybrid classes
We usually run two cohorts per year, one in the Spring (April to July) and one in the Fall (September to December).
